In my case, the post on the end of the carabiner wore down and came right out of the buckle, sending camera and lens to the ground.  Your Perlon rope safety cord, the way you show in the picture, would not prevent dropping the camera in that situation unless the rope went through the buckle or around the strap.  It does cover the problem of the bolt coming out of the camera which is the most common cause of failure I'm told.  The folks at Black Rapid have promised to send me a new strap with a tether system they have developed so it will be interesting to see what they have come up with.  Thanks for your suggestions.

Also note the safety cord, which is a short piece of 5mm Perlon, a material used in climbing ropes.  All my cameras have such a loop and I use them with regular straps as well .. never know, and they're much stronger and longer-lasting than shoe laces :).

If you're wondering what I do when I want to use the vertical camera orientation, the RRS screwclamp is easy to remove, even with gloves, and slides out of the way.  The Perlon loop keeps it handy for replacement.  A photo of same:  https://flic.kr/p/NsY3i1.

This arrangement, of course, would only be appropriate for those Arca-Swiss tripod heads, but hopefully some suggestions, especially the Perlon loop, would be applicable to your own camera strap arrangements.
