On a similar note, I carry a 500mm on a monopod.  Once the camera came loose... luckily I noticed it before it dropped off... I must have hit the release button.  I now have a safety strap from the camera to the lens, so if it happens again, the body will just hit me and not the ground.

In my case the metal pin at the end of the carabiner had worn down enough to come right out of the buckle on the strap.  Fortunately the camera and lens fell on soft ground and there was no damage.  These straps are great but can fail, so a back-up system to prevent catastrophe is mandatory!

I checked my strap and noticed the Carbineer has worn through by about 60 percent, so I added a shoelace to go from the tripod mount to the strap as a safety back up.
 
So because I care, I would suggest you check your straps for wear so you do not damage your camera and lens.
